1) Showing up for your
appointment on time and you didn't miss your last appointment.
2) No loose bands or brackets or broken appliances.
3) Good oral hygiene or a great improvement.
4) Wearing your elastics as instructed.
5) Wearing your headgear, lip bumper, or appliance as instructed.
WRITE YOUR NAME ON THE BACK OF EACH
TICKET AND PUT THE TICKETS IN THE PLASTIC TUBE.
We'll draw 5 names from
